BCOP Updates 2026: Stepping-Up Your Game: Outpatient Myeloma Bispecifics
Multiple myeloma is a clonal plasma cell disorder that can lead to end organ damage. After initial treatment, patients will enter a period of remission. Unfortunately, this is an incurable hematologic malignancy, thus all patients will relapse with subsequently shorter remissions until they are considered relapsed refractory. Novel therapies such as bispecific antibodies and trispecific antibodies, harness immune effector cells to target clonal plasma cells leading to unpredecented levels of efficacy. However, this can also lead to a state of hyperinflammation, known as cytokine release syndrome. To mitigate this, patients are administered step up doses (SUD) with premedications while admitted to an inpatient facility. Experience in both real-world settings and clinical trials have led to the adoption of outpatient programs. This improves the reimbursement burden, saves inpatient beds, and allows for treatment under the supervision with a familiar team.
